We prescribe Dexcom G7 or Abbott FreeStyle Libre 3 Continuous Glucose Monitors (CGMs) and also provide cellular blood glucose meters for patients who are not eligible for a CGM.
No, using a Continuous Glucose Monitoring (CGM) device is optional. While a CGM can provide real-time tracking of your glucose levels, we can monitor your glucose data with a blood glucose meter. Our healthcare team will work with you to determine the best approach for managing your diabetes.
Yes, if you are wearing a FreeStyle Libre or Dexcom device we can easily connect the data from your sensors to our platform giving you access to a dedicated care team.
You will be paired with a health coach who will check in with you monthly via phone. Together, you will create a comprehensive care plan that provides you and your providers with quick access to your medical history, conditions, and health goals. This plan will include specific strategies to help you achieve your health objectives.
During your monthly calls, you’ll have the opportunity to discuss any changes in your health, care plan, or lifestyle. Your health coach can address any concerns and work with your care team to ensure everyone is updated on your progress. They will regularly reach out to discuss any health issues you may have, such as your exercise habits or medications. The coaching team has extensive experience in helping patients set and meet their health goals, and your coach will track your progress along the way.
